About the Transition to adulthood Service

Our Transition to adulthood Service supports young people aged 16-25 who are preparing to leave care or who have already left care.

We take immense pride in our role as corporate parents to the children and young people in our care, as well as those who have left our care. We recognise that our responsibility is to provide each child with the same level of care, respect, and support that we would want for our own children. Our commitment is to champion their successes, celebrate their achievements, and ensure they have the robust support needed to thrive in every aspect of their lives.
