Community Support Worker

Job summary

An exciting opportunity has arisen to employ a full-time Community Support Worker within the Neighbourhood Mental Health Team. This role will focus on the delivery of population-based mental health care for the community of Droitwich, based in our new suite within Droitwich Medical Centre.

You will be working alongside a team of highly specialist practitioners and have direct relationships with the professionals within the local Primary Care Networks (PCN).

Please note the Trust will not routinely provide sponsorship to work for this role. Employment on a time limited visa doesn't guarantee sponsorship at point of expiry. Main duties of the job Will work as a key member and resource of the MDT core team.

Aspects of the role would also include close working within the MDT, undertaking and supporting individuals accessing the service in Recovery focussed activities. There is scope for professional development as the trust has a repertoire of training available to those wishing to extend their skill set.

Support registered professionals to deliver evidenced based interventions, managing a caseload of patients Supporting and signposting to improve social isolation and improved population health.

You will have experience of working with this user group in community settings. The ability to work flexibly as the need of the service demands is essential. Attend weekly MDT meetings, participate in professionals meetings, signpost to adjunct services depending on patient need.

Support and guidance to the carers of patients accessing the services.

Work across all clinically appropriate patients, 17-years-old and over, to deliver evidence-based interventions. Provide support to MDT colleagues in delivering interventions. Work as integral team member, bolstering recovery-based interventions in the community.

Person Specification

Skills and Abilities Essential Ability to challenge poor behaviour. Role modelling good behaviour. A positive attitude and able to act on feedback. Ability to praise and be supportive to others. Ability to communicate clearly and effectively in English in both verbal and written formats.

Qualifications

Essential Level 3 qualification (e.g., A-level, NVQ3) in Care or equivalent qualification or experience. Level 2 qualification (e.g., GCSE, NVQ2) in Care or equivalent qualification or experience and will be expected to undertake the relevant level 3 qualification on appointment to post.

Will be expected to meet the Care Certificate standards within 12 weeks of commencing employment with the Trust. I understand that I will be asked to provide original certificates to verify my qualifications at point of offer. Experience Essential Previous relevant experience working with the client group.

One to one working. Personal care tasks. Desirable Working with adults/older adults with mental health problems.

Additional Criteria Essential Applicants should be able to work flexibly and with minimal supervision. The ability to cope with changing situations to routine is also important. Able to meet the travel requirements of the post.
