Care Manager

Job DescriptionThe Central North Aberdeenshire Care Management Team are a busy and committed team who focus on Rehabilitation and Enablement, Self-directed Support assessments, and the planning and commissioning of care and support for older adults and those with a physical disability, including an acquired brain injury. We require an equally committed individual with appropriate experience, knowledge, and skills to fill a permanent vacancy within our team.

The role will include working in partnership with families and carers, relevant professionals, and other agencies, whilst adhering to all applicable legislation, such as the Social Work (Scotland) Act 1968 and the Social Care (Self-directed Support) (Scotland) Act 2013, as well as Aberdeenshire Council policies and procedures. Applicants will also be required to undertake work that arises from the Adult Support and Protection (Scotland) Act 2007.

The office base is in Ellon; however, the successful candidate will be expected to work across the Central locality.

There is a responsibility for the post holder to demonstrate a commitment to quality service delivery through continuous improvement for the benefit of the Service and the organisation.
