"What if your advanced clinical assessment could prevent a patient from spending the night in hospital?" Join our countywide Evening & Night Community Nursing Service in Worcestershire, currently based at Droitwich.

We are seeking an enthusiastic, innovative and clinically confident ACP to deliver advanced assessment, clinical decision-making and treatment to adults with complex, urgent and changing healthcare needs.

Working evenings and nights alongside experienced community nursing teams and urgent and emergency care partners, you will support admission prevention, ambulance avoidance and safe care closer to home.

You will also attend some daytime service meetings and build strong working relationships with Hospital at Home, UCR and Neighbourhood Teams. Main duties of the job As our ACP, you will provide senior clinical expertise and leadership within the Evening and Night Service, supporting nurses and wider teams to safely manage increasingly complex patients in their own homes.

You will: Undertake advanced clinical assessments, physical examinations and diagnostic reasoning for complex and urgent presentations. Prescribe within your scope and use appropriate PGDs, request and interpret investigations, and develop treatment plans to support safe care at home.

Assess and manage patients at risk of hospital admission, supporting admission avoidance and safe escalation or transfer of care. Provide expert clinical advice to community nursing colleagues and work collaboratively with GPs, urgent community response, ambulance, acute and wider health and social care services.

Initiate and review ReSPECT documentation where appropriate. Provide clinical leadership, education, supervision and professional support to colleagues. Contribute to service improvement, innovation, audit and research, ensuring opportunities support the four pillars of advanced practice alongside clinical responsibilities.

A role where you can really make a difference. Evenings and nights can be challenging times for patients, families and professionals. Access to senior clinical decision-making can make the difference between a patient being admitted to hospital and receiving the right intervention safely at home.

We want our ACP to help us bridge that gap. You will be able to bring advanced clinical expertise directly into the community, supporting nurses to manage complexity and providing an additional level of clinical assessment and decision-making when other services may be less accessible.

Qualifications

Essential Registered Nurse Level 1 / Allied Health Professional / Paramedic / Pharmacist MSc Advanced Clinical Practice Non-Medical Prescribing (V300) / ability to work within PGDs, within scope of professional practice Advanced Assessment Skills Module Evidence of a portfolio of knowledge, skills and professional development Desirable Mentorship or teaching qualification / work place assessor Qualification(s) in long term conditions Leadership qualification Experience Essential Demonstrable post-graduate experience in a relevant area Extensive post-graduate experience in a role undertaking assessments of complex needs Experience of working with people with long-term illness and / or older people Clinical experience of caring for people with cognitive impairment Experience of working across professional boundaries Experience of change/leading service development Experience of clinical leadership Knowledge Essential Extended and advanced clinical skills and knowledge base Understanding of professional and current issues in relevant clinical area Demonstrate an understanding of clinical governance Understanding of relevant legislation and guidance relating to clinical area Understand the framework for working at an advanced level Knowledge of theory behind change management Skills and Abilities Essential Ability to communicate clearly and effectively in English in both verbal and written formats Ability to constructively challenge and articulate clinical opinion Ability to prioritise workload effectively Evidence of leading and managing a team High level problem solving skills Teaching skills Able to make decisions autonomously Ability to influence and negotiate Able to meet the travel requirements of the role.
