Practice Nurse - North Cardiff Medical Centre

Job summary

The Practice Nurse will provide: assessment, treatment, screening and health promotion services and advice working in accordance with the NMC Code of Conduct and within their scope of practice. nursing services to patients independently or in participation with Nurse/GP colleagues working to agreed protocols and guidelines.

Main duties of the job Clinical Practice Provide clinical services as designated by the Nurse Manager/Clinical Governance Lead and Provision of practice Nursing duties Work with other health care professionals to monitor, manage and treat long-term conditions, in line with national and local policies and practice needs.

Review medication for therapeutic effectiveness, appropriate to patient needs and in accordance with evidence-based practice and national and practice protocols.

Work with patients in order to support adherence to prescribed treatments Support patients to adopt health promotion strategies that promote them to live healthily, and apply principles of self-care Support and manage health needs of women presenting for family planning and cervical cytology consultations.

Participate in vaccination and immunization programmes for both adults and children. Undertake immunisations, injections and vaccinations Undertake screening, e.g. blood pressure, cervical cytology.

Management of medical/nursing supplies, including stock control and expiry checks. Maintenance of treatment room and clinical equipment including testing and recording where so defined and adherence to infection control procedures.

Maintain accurate comprehensive records of all consultations and treatments in the patient record and completion of all necessary documentation associated with patient health care.

Qualifications

Essential RGN with current PIN number Practice Nurse experience Membership of a professional body e.g RCN / MDU Desirable Post-basic Nursing qualification -ECDL or equivalent. -Professional knowledge acquired through training to diploma/degree level Appropriate training /experience in chronic disease management and other training as identified by their manager Additional Criteria - ESSENTIAL Essential Knowledge of accountability of own role in a nurse-led service Knowledge of health promotion strategies Knowledge of patient group directions and associated policy Communication skills, both written and verbal Ability to communicate difficult messages to patients and families Able to operate in a changing environment An effective team player-flexible, adaptable and versatile.
