At Cruddas Park and Hillsview, pharmacists are valued clinicians at the heart of patient care. We are proud to have an experienced pharmacy team comprising two full-time Clinical Pharmacists and a full- time Pharmacy Technician, working closely with GPs, nurses and the wider multidisciplinary team.

Main duties of the job We are looking for a pharmacist who shares our belief that excellent healthcare means looking beyond the medication list and understanding the whole person sitting in front of you.

You'll have the opportunity to: Manage your own clinical workload. Develop expertise in long-term condition management. Undertake meaningful Structured Medication Reviews.

Build ongoing relationships with patients. Influence prescribing quality and medicines optimisation. Shape new initiatives and pursue your own clinical interests.

The Role This practice-based role is for 22.5 hours per week. Your week will be varied and typically split between: Planned Clinical Sessions Structured Medication Reviews(SMRs). Long-term condition management.

Patient-initiated medication reviews. Medicines optimisation. Duty Pharmacist Sessions Reviewing acute prescription requests.

Responding to medication queries from patients and clinicians Supporting safe prescribing across the practice. Working collaboratively with colleagues to resolve medication-related issues.

You'll receive regular support from our Lead Pharmacist and GP Clinical Supervisor, with dedicated pharmacy and practice meetings for case discussion, learning and professional development. MainDuties Provide expert medicines advice to the multidisciplinary team. Conduct Structured Medication Reviews and medication optimisation reviews.

Manage patients with long-term conditions including: Atrial Fibrillation Chronic Kidney Disease Heart Failure Gout Hypothyroidism Hypertension Support nursing colleagues with the management of diabetes and respiratory conditions. Review acute prescription requests.

Support medicines reconciliation following hospital admissions and out patient appointments. Help the practice achieve local and national prescribing quality targets. Undertake medicines optimisation and quality improvement projects.

Support vulnerable patients and help reduce barriers to accessing health care and medication. Maintain awareness of CQC and QOF medication safety requirements.

Person Specification

Knowledge and Skills Essential Strong clinical knowledge and decision-making skills. Other Essential Is passionate about holistic, patient-centred care. Enjoys managing complex patients and long-term conditions.

Works collaboratively with in a multi disciplinary team. Is proactive, compassionate and motivated to improve patient outcomes. Wants to continue developing professionally with in primary care.

Qualifications

Essential MPharm Degree. Ongoing evidence of Continuing Professional Development (CPD). Completed, or willing to undertake, the CPPE Primary Care Pharmacy Education Pathway.

Desirable Independent Prescribing Qualification.
