About the Role
We are seeking an experienced, motivated, and compassionate Assistant Team Manager to join our Hospital Discharge Team. This is an exciting opportunity to support the delivery of safe, effective, and timely hospital discharges while promoting independence and positive outcomes for individuals leaving hospital.
Working within a fast-paced multidisciplinary environment, you will support the Team Manager in providing operational leadership, professional supervision, and oversight of complex discharge planning. You will work closely with health partners, social care colleagues, community services, care providers, individuals, and their families to ensure person-centred and strengths-based practice.
Key Responsibilities
- Support the Team Manager in the day-to-day leadership and management of the Hospital Discharge Team.
- Provide supervision, guidance, and support to social workers, care coordinators, and support staff.
- Oversee complex hospital discharges and ensure effective risk management and decision-making.
- Promote high-quality strengths-based assessments and person-centred discharge planning.
- Ensure compliance with relevant legislation, policies, and professional standards.
- Support staff performance, development, and wellbeing through regular supervision and coaching.
- Work collaboratively with NHS colleagues and partner agencies to facilitate safe and timely discharges.
- Monitor service performance, quality standards, and key performance indicators.
- Contribute to service improvement initiatives and promote best practice across the team.
- Support safeguarding activity and ensure appropriate responses to adults at risk. About You
We are looking for someone who has:
- Significant experience working within adult social care, hospital discharge, or community services.
- Experience of supervising or mentoring staff within a health or social care setting.
- Sound knowledge of the Care Act 2014, Mental Capacity Act 2005, safeguarding, and hospital discharge processes.
- Strong assessment, risk management, and decision-making skills.
- Excellent communication and partnership-working abilities.
- The ability to manage competing demands within a busy and complex environment.
- A commitment to delivering high-quality, person-centred services. Essential Requirements
- Qualified Social Worker, with relevant professional qualification.
- Current professional registration where applicable.
- Experience of working within a multidisciplinary environment.
- Ability to travel across the service area as required. What We Offer
