What testing strategy actually catches real bugs?

We have eighty percent code coverage but production bugs still slip through constantly. The tests that catch the most issues are the ones nobody wrote. What testing approaches have actually reduced your production incident rate?

I think code coverage is just one piece of the puzzle, we've found that testing for specific business scenarios and edge cases with tools like Cypress has been way more effective at catching bugs that actually matter to our users.
