An exciting opportunity is available for a caring and ambitious School Nurse to join the school nursing team as part of our established integrated Warrington 019 Public Health Nursing team.

You will work with children, young people, families and schools, managing a varied caseload and delivering high-quality, evidence-based care, with dedicated support from Team Leaders and the Senior Management Team. Main duties of the job You will: Be a qualified School Nurse with the Specialist Community Public Health Nursing qualification, or due to qualify within the next six months. Provide professional leadership and support within the integrated 019 team, contributing specialist school nursing knowledge and expertise.

Work autonomously to assess health needs, plan and deliver evidence-based interventions, manage a defined caseload and respond effectively to the needs of children, young people, families and schools.

Deliver the Healthy Child Programme for school-aged children, including health promotion, early intervention, targeted support and contribution to service development and innovation. Demonstrate a flexible, organised and conscientious approach, with the ability to prioritise workload and respond to changing service demands.

Communicate effectively with children, young people, parents, carers, education settings and partner agencies, promoting the service and working in the best interests of the child through a whole-family approach.

As a key professional in safeguarding children and young people, you will identify and respond to risk, work collaboratively with education, social care, health and other agencies, and contribute to multi-agency plans that promote safety, wellbeing and positive outcomes.

To work as the named School Health Nurse for an identified High School, college and a cluster of Primary schools. To manage the defined caseload, using a public health approach to provide and maintain a high-quality service that promotes the physical and mental health of the school aged population and their families.

To lead to the delivery of the Healthy Child Programme 519 and co-ordinate the delivery of public health interventions for the post holders assigned schools under the direction of the 0-19 Team Leader.

This will include health screening/surveillance health assessment, immunisations, and a requirement to support the health needs of any vulnerable children/young people within the caseload. Work in collaboration with partnership agencies to address the needs of vulnerable children including those on the child protection register. Duties and

Responsibilities:

1. The named School Nurse will be responsible for co-ordination of the School Health Plan with their named schools on an annual basis. 2.

Contribute to an integrated Public Health Nursing Service by linking to education settings and other partner agencies 3.

Lead on the recommended health assessment and reviews including: Reception/Year 1 (age 45) school entry assessment (transition from 05 HCP school entry questionnaire; Providing Year 6/7 (age 1012) assessment at transition from primary to secondary school. 4.

Supporting mid-teens reviews, when young people are embarking on the next transition stage as and when required in national and local specifications. 5. Lead and coordinate a core programme of interventions according to agreed procedures, ensuring that Trust targets are met. 6.

Undertake comprehensive health assessments for identified children and young people, including annual health assessments for those in local authority care in line with the Healthy Child Programme 4-19 years of age. 7. To contribute to the delivery of the national immunisation programme for school aged children. 8.

Plan, implement and evaluate interventions according to changing health needs to improve health outcomes. Consult, advise and educate children, young people and families to enable them to effectively manage their health needs. 9.

Perform home visits to children and families advising on all aspects of health as appropriate. 10. Working in partnership with other key stakeholders to safeguard and protect children and young people. 11.

To contribute and take part in regular team meetings and contribute to multi-agency groups as appropriate. 12. Working collaboratively to support children and young people where there are identified health needs, or where they are in the child protection system, providing therapeutic public health interventions for the child and family 13.

Provide training where necessary to young people, parents, teaching personnel and other professionals. 14. Provide signposting to other services and agencies, referring and liaising appropriately. 15.

Take and act upon referrals from other agencies and services, as appropriate. 16. Provide a confidential support and advice service; that is easily accessible, for young people within a high school or other venue on a regular basis. 17.

Organise, facilitate and participate in health promotional and health educational activities in schools and community settings, working in partnership with school staff and with other statutory and voluntary agencies. 18. Utilise opportunities to promote healthy lifestyles, positive mental health and the public health agenda. 19.

Ensuring children not in employment, education or training, or children educated at home receive the universal offer. 20. Complete PDRs and return to work pro-forma as required to the designated named staff within the team. 21.

Ensure performance data is collated in a timely manner Clinical Maintain active status on NMC register acting in accordance with the professional code and guiding documents in adherence with Revalidation Framework.

Develop a working knowledge and experience of IT systems Maintain contemporaneous, factual records in accordance with Trust guidelines and policies. Be aware of current working practices in accordance with Trust policies and procedures. Manage their time effectively to ensure prioritisation of workload in order to achieve agreed deadlines.

Be responsible for own professional updating and keep abreast of current research based practice. Participate in study days as appropriate and be aware of current developments and research within the area of practice. Observing the complaints and disciplinary procedures in accordance with the Trust policy.

Report sickness absence and return relevant forms to line manager. Maintain portfolio of professional development. Leadership Provide supervision, where appropriate to support the skill mix team Undertake appropriate delegation within team.

Support other School Nurses and staff within the team, including the induction of new staff members. Ensure effective learning experience and opportunities to achieve learning outcomes for skill mix team.

Develop specialist knowledge and expertise in an area of practice appropriate to service needs and, act as a resource to colleagues and other professionals in the chosen area e.g. Asthma, Behaviour Management. Allergy etc.
