Job summary

This role is being advertised on behalf of Littleborough Group Practice and the successful applicant will be directly employed by Littleborough Group Practice. Main duties of the job The Practice Nurse will provide high quality, patient centred care in a primary care setting.

The post holder will undertake routine and complex clinical tasks, deliver preventative health interventions, and manage long term conditions. The nurse will work within the multidisciplinary team to ensure comprehensive patient care and support practice objectives.

Responsibilities

Conduct adult and child immunisations including COVID-19, influenza, MMR, pneumonia, RSV, shingles, whooping cough and other vaccines as per national guidelines. Perform blood tests, ECGs, blood pressure monitoring, and routine physical health assessments.

Perform urinalysis testing and act on results as part of routine and chronic disease assessments. Administer injections including B12, Decapeptyl, Nebido, Depo, Inclisiran, and Prostap injections. Provide contraception services including contraceptive pill checks and Depo Provera (contraceptive) injections.

Undertake chronic disease management including asthma reviews, COPD reviews, CKD and CHD reviews, diabetes reviews, hypertension reviews, mental health reviews, learning disability reviews, and rheumatoid arthritis management. Conduct new patient medicals and NHS Health Checks. Perform cervical smears, spirometry, and swabs as required.

Deliver health promotion including smoking cessation advice, weight and BMI monitoring, and lifestyle support for self-management of chronic conditions. Deliver patient education and support for self-management of chronic conditions. Identify and escalate patient deterioration and safeguarding concerns to appropriate staff.

Maintain accurate and contemporaneous patient records in line with professional and practice standards. Professional Responsibility Maintain current registration with the Nursing and Midwifery Council. Work in accordance with the NMC Code and professional standards.

Undertake ongoing continuing professional development to maintain clinical competence. Comply with infection prevention and control policies and safety requirements.

Person Specification

Person Spec Essential Current, unrestricted registration with the Nursing and Midwifery Council (NMC). Experience of practice nursing or primary care nursing. Competent in cervical cytology, immunisations and injection techniques.

Experience of chronic disease management and long term condition reviews. Proficient in the use of the EMIS Web clinical system, including consultations, clinical coding, searches and recalls. Sound working knowledge of QOF and NHS primary care contractual frameworks, and the ability to deliver against them efficiently.

Ability to work autonomously and as part of a multidisciplinary team Desirable Spirometry and/or advanced respiratory review competency.
